Shipping Destinations
Form K9 ships to all 50 states within the United States of America, including Alaska and Hawaii. We currently do not offer international shipping. All orders must be shipped to a valid US street address. We accept PO Box addresses for small, lightweight items only. Crates, pens, modular kennels and other large structural products must be delivered to a physical street address capable of receiving freight delivery.
Risk of Loss
All orders placed with Form K9 are made pursuant to a shipment contract. This means that risk of loss and title for items purchased from Form K9 passes to you upon delivery of the item to the carrier. Form K9 is not responsible for lost or damaged packages once they have been accepted and scanned by the carrier. However, we will provide reasonable assistance in filing claims with the carrier on your behalf at no charge. All shipments include a minimum of $100 in carrier-provided liability coverage. Additional insurance is available at checkout.
Carrier Selection
Form K9 selects the shipping carrier at our discretion based on the size, weight and destination of your order. Carriers we use include United Parcel Service (UPS), Federal Express (FedEx), United States Postal Service (USPS) and regional LTL freight carriers for oversized items. If you have a preference for a specific carrier, please note this in the order comments field — we will accommodate where possible but cannot guarantee carrier selection.
Delivery Estimates
Delivery estimates provided at checkout are based on carrier transit time tables and do not account for processing time, carrier delays, weather events, natural disasters, labor actions or other circumstances beyond our control. Form K9 does not guarantee delivery by a specific date unless you select a guaranteed overnight or two-day service and your order is placed before our daily cutoff of 2:00 PM Central Time on a business day.
For freight shipments, the carrier will contact you to schedule a delivery appointment. You must be present at the delivery address to receive freight shipments. If you miss a scheduled freight delivery, re-delivery fees charged by the carrier will be your responsibility.
Order Processing
Orders are processed on business days only — Monday through Friday, excluding US federal holidays. Orders placed after 2:00 PM Central Time are processed on the following business day. Orders placed on weekends or federal holidays are processed on the next business day. Processing time is separate from and in addition to carrier transit time.
Address Accuracy
You are responsible for providing an accurate and complete shipping address at the time of purchase. Form K9 cannot be held responsible for delivery failures or additional carrier fees resulting from an incorrect or incomplete address provided by the customer. If an address correction is required after shipment, carrier address correction fees will be charged to your original payment method.
Free Shipping Eligibility
Orders totaling $99.00 or more (after discounts, before tax) qualify for free standard ground shipping to continental US destinations. Free shipping is not available for Alaska, Hawaii, oversized/freight items, or items requiring special handling. Free shipping orders are shipped via our choice of carrier using the most economical method.
